From our main page you can click the quote/information button. The next menu will offer a “smart wizard”. Just answer a few questions about what is important to you for a health plan and the wizard will suggest the best plans. What information do I need to apply
online or by phone?
The application has questions about the applicants name, address, date of birth, phone #, occupation, employer, medical conditions and history, prescriptions, tobacco use and other information needed to decide on your application. Please call our toll free number if you need any assistance with the application.
What if I prefer to fill out a
paper application?
Just call our toll free number and we will send you a brochure and paper application.
What is the application cost?
If you apply by phone or at our office the application fee is $25. If you apply online there is no application fee. (This is subject to change by Unicare.)
Does my coverage start immediately
when I apply?
No. Your coverage will not start until Unicare notifies you that you are approved and that your policy is in force. We strongly suggest that you don’t cancel any existing policy until your new policy is effective!
What are my payment method options?
You can pay monthly by checking account deduction or by 3- month (quarterly) billing.
